Local: obstrucción nasal, epistaxis, inflamación/destrucción hemifalcial por extensión a tejidos locales.
Afectacion MO infrecuente: Complicacion, SME hemofagocitico.
DD Los linfomas T o B (no tienen relación con VEB) Granulomatois linfomatoide Granulomatosis Wegener

NEOPLASIAS LINFOMA NK: Histologia: Ocupacion, Infiltracion, Ulceración
Arquitectura: Hiperplasia pseudoepitoliomatosa. Patron angiocentrico y angiodestructivo. Pleomorfismo (celulas de tamaño variable) Anaplasia Nucleo a menudo plegado con cromatina granular citoplasma claro con granulos azurofilos semejantes a los de las células NK
normales. Mitosis frecuente. Necrosis y apoptosis
Marcadores inmunologicos: CD2+, CD56+, surface CD3−, with cytoplasmic CD3ε+. Cytotoxic molecules
(granzyme B, TIA-‐1, and perforin) Other T-‐ and NK-‐cell–associated antigens, usually negative: CD4, CD5, CD8,
TCRδ, CD16, and CD57.
Con FIS se comprueba en todos los casos asociación con VEB.
Alteraciones cromosomicas no caracteristicas: 6q−, 2q+, 8p−, 11q−, 12q−, 13q−, 15q+, 17q+, and 22q+.

NEOPLASIAS LINFOMA NK: Tratamiento
Formas precoces: QT + RT Formas avanzadas (Est III, IV):
Multi QT (L-‐asparaginase-‐containing regimens) TMO autologo ( potentially curative option ….is yet to be clearly defined
Indice pronostico: invasion of bone or skin high-‐circulating EBV DNA levels presence of EBV-‐positive cells in the bone marrow. (niveles DNV-‐VEB correlacionan con extensión y respuesta al tratamiento)
Sobrevivencia a 5 años: 30-‐40% inicialmente hasta 70% con RT + tratamiento intensivo.
La QT no mejora la sobrevivencia. Muerte por progresión a pesar de tratamiento agresivo.
