Hipertensión Resistente: Definición. Por convención se considera hipertensión

refractaria o resistente cuando resulta imposible disminuir la presión arterial (PA)

por debajo de 140/90 mm Hg. con un tratamiento que incluya: una combinación

de 3 fármacos antihipertensivos de diferentes mecanismos de acción,

en dosis adecuadas e incluyendo un diurético tiazídico.

Consenso: HTA mayor de 140/90 a pesar de estar recibiendo tres o más

antihipertensivos en dosis apropiadas, de los cuales uno debe ser un diurético.

Conclusiones

•  La resistencia a la terapia antihipertensiva puede persistir por años, aún aplicando un manejo agresivo.

•  La HTR se asocia a un alto riesgo de AVE y a daño de órganos blancos.

•  Aparentemente existe una asociación entre HTR, Obesidad y Diabetes mellitus. Pudiese la hiperinsulinemia participar en el fenómeno de resistencia.

HTA Resistente por Exceso de Sal

No pasemos por alto que un consumo excesivo de sal, puede conducir a una expansión del

volumen plasmático e HTA de difícil control.

En aquellos pacientes cuya historia sugiere poca adherencia a la restricción de sodio:

una muestra de orina de 24 horas con medición de sodio urinario puede proporcionar información valiosa. Valor deseado < 100 mEq/día. Bakris 2008 JACC

Tradicionalmente en los estudios clínicos se había utilizado el

esfingomanometro de mercurio para la medición de la presión

arterial, pero por primera vez en el estudio ASCOT se utilizó un equipo

automático.


 Deben ser de brazo, NO DE MUÑECA.

 Deben chequearse 1 a 2 veces por año.  Aparatos certificados: www.bhs.org  Sí en la mayoría de los pacientes (autocuidado),

pero NO en aquellos con ansiedad anormal.  Protocolos de mañana y tarde en días fijos o

protocolos de 4 dias seguidos.  A futuro sistema “parcialmente ciego” al paciente.

  Es la primera posibilidad a plantearse ante un paciente con un proceso crónico y habitualmente asintomático como la HTA.

  El mal cumplimiento terapeútico debe siempre descartarse en pacientes que no consiguen control de la HTA.

  Para ello, disponemos en la clínica de unas herramientas sencillas, como son los test de Batalla y Morinsky Green, test validados, que mediante el planteamiento al paciente de un escaso número de sencillas preguntas nos permite evaluar el grado de adherencia al mismo.
