La clasificación molecular orienta las opciones de tratamiento
Implicaciones clínicas de la sobre-expresión de HER2/neu
• La proteína ErbB2 ó HER2/neu se encuentra amplificada en un
20% a 25% de cáncer de mama
• Es un factor pronostico adverso tanto en estadios iníciales
como avanzados de la enfermedad
• Es también un marcador predictivo de eficacia de terapias
dirigidas frente a la propia proteína
• Es mandatorio su determinación en todo nuevo diagnostico –

TRASTUZUMAB en ADYUVANCIA en CM Her2 (+)
• Trastuzumab es una sólida realidad en el tratamiento adyuvante del CM Her2 (+). • Un año de trastuzumab reduce el riesgo de muerte en un tercio de las pacientes
• Beneficio de Trastuzumab en todos los subgrupos analizados ( GP, GN)
•Nuevas combinaciones con Trastuzumab muestran beneficios en algunos subgrupos de pacientes
•Pendiente de los resutados de SG en el estudio en combinación con Neratinib

Conclusiones Neoadyuvancia
• Tto neoadyuvante con doble bloqueo aumenta la tasa de RCp – La tasa de pCR global con doble bloqueo es >60%, pudiendo llegar al 95% en RH(-)
• La combinación de quimioterapia + trastuzumab + pertuzumab es la que obtiene mayor tasa de RCp con mejor perfil de toxicidad
• Lapatinib o neratinib no indicado como parte del Tto neoadyuvante
• PAM50 puede identificar subtipo HER2enriched, que obtiene mayor tasa de RCp con esquemes sin Qt

Conclusiones
• Tratamiento anti Her 2 ha supuesto revolución en el tratamiento del cáncer de mama HER2+
• Mejoría de SG
• Papel reseñable de las combinaciones en la neoadyuvancia y quizás en la adyuvancia
• Nuevas posibilidades de combinación
• Aún queda por hacer…
