OBJETIVOS Y EVALUACIÓN TRATAMIENTO
OBJETIVO: curar la infección para prevenir cirrosis, descompensación, CHC,
manifestaciones extrahepáticas severas y la muerte (A1)
ARN-VHC indetectable (≤15 UI/ml) a las 12 semanas (RVS12) y 24 semanas
(RVS24) tras tratamiento (A1)

GENETIC BARRIER & PERSISTANCE
• En el momento actual no se puede hacer una recomendación general para
estudiar resistencias basales
• Sin embargo, en pacientes con genotipo 1a con cirrosis, es necesario
determinar el polimorfismo Q80K para el inicio o rescate de pautas basadas
en Simeprevir (A1),
• y para emplear la combinación Grazoprevir/Elbasvir sin ribavirina y
durante 12 semanas, en cirróticos con genotipo 1a previamente
respondedores nulos es necesario descartar la presencia de RAVs
asociadas a resistencia a Elbasvir (A1)

Alternatively, wait until alternative therapeutic options become

Close monitoring (safety) is required as these regimens are off-label

manejo de la Hepatitis C
• Las pruebas de resistencia se deben utilizar para guiar el cambio de
tratamiento en aquellos pacientes que no puedan esperar a la llegada de
nuevos regímenes de tratamiento. Si no existe una necesidad urgente de
retratamiento, la mejor estrategia es diferirlo hasta que las nuevas
combinaciones estén disponibles (A1)
• Si no se dispone de una prueba de resistencias, las estrategias de
retratamiento han de incluir necesariamente fármacos sin resistencia
cruzada y esquemas con ribavirina y de mayor duración (A2)

RECOMENDACIONES FINALES
Éxito del tratamiento con AAD depende de factores
asociados con el virus, los fármacos y el huésped.
Determinar el genotipo sin cometer errores.
En el tratamiento de infecciones por genotipos 1a y 3 las
RAS en NS5A tienen impacto en determinados grupos.
Estudio de RAS basal permite optimizar tratamiento de
forma personalizada (duración, con/sin ribavirina).

RECOMENDACIONES FINALES
La primera opción de retratamiento es diferirlo si
el paciente puede esperar.
En caso de requerir retratamiento:
Repetir genotipado para valorar reinfección o recaída.
Realizar estudio de resistencia para guiar re-tratamiento.
Aumentar duración + Ribavirina.
Cambiar familia AAD
Utilizar SOF combinado con 2-3 fármacos.
