Adaptación de un instrumentoneuropsicológico para la evaluación

La evaluación del funcionamiento cognitivo es fundamental no sólo para el diagnóstico decondiciones patológicas, sino también para el manejo medico y conductual de los
pacientes.
La falla en los distintos tipos de memoria, es un déficit importante en los pacientes que
presentan demencia.
El objetivo de este estudio de adaptación de un instrumento de evaluaciónneuropsicológica de Ia memoria, fue darle énfasis a las operaciones de encodaje yrecuperación de Ia información, ya que numerosos trabajos mostraron que las operaciones
de encodaje, las condiciones de recuperación y Ia interacción afectan de formadeterminante Ia performance mnesica.
El tamaño de Ia muestra se estableció en 20 individuos. Los sujetos fueron evaluados,previamente, descartando a todas aquellas personas que presentaran índices de posibles
alteraciones psiquiátricas (Psicosis, DOC, alcoholismo, drogadicción, depresión), AVE oalgún indicador de demencia.38
Se mantuvo Ia equiparidad de sexos, con nivel socioeconómico heterogéneo y escolaridadmínima de cuatro años (periodo necesario para adquirir las funciones de lecto -escritura) y
un rango etario de 50 a 80 años.
Los resultados obtenidos indican que la traducción al castellano y adaptación a partir del
manual de test de Procedimiento de Recuerdo Libre de Grober y Buscke, comparte lasventajas y resultados obtenidos por los investigadores belgas.
Si bien este estudio se constituyó como una investigación exploratoria, con una muestrapequeña, sin significancia estadística, cabe destacar, que los resultados obtenidos sonmuy similares a los presentados por el test belga, lo que alude a un adecuado procesometodológico.
Hemos revisado la bibliografía que nos ha presentado evidencias experimentales, queconfirman la visión general que el desempeño de la memoria si disminuye como una

función del envejecimiento normal, pero que disminuye más en algunas situaciones que en
otras. En efecto, se ha demostrado que el recuerdo inmediato de una secuencia corta deítems disminuye muy poco con la edad, pero que las pérdidas esenciales se encuentran en
la memoria de trabajo. En este último caso, el sujeto no sólo debe mantener la informaciónen la mente sino que también debe manipularla, organizarla o combinarla con mayor
información que entra. En las situaciones de la memoria a largo plazo, las personasmayores muestran típicamente disminuciones del rendimiento aunque aquí también, el
tamaño del efecto, depende de la tarea o de sus condiciones.
Por otro lado, los deterioros relacionados con la edad más significativos se encuentran en
las tareas que proporcionan pocas guías para codificar las operaciones y poco apoyo en larecuperación. Sin embargo, los deterioros disminuyen siempre y cuando se presente, en el
momento de la adquisición, instrucciones (por ejemplo el generar elaboradores precisos, orealizar pequeñas tareas motoras), y éstas sean combinadas con trabajos de apoyo, como
por ejemplo, un recuerdo con claves o reconocimiento en el momento de la recuperación.
Con respecto a la aplicación del test en los diferentes grupos etarios, se observó que éste
era un instrumento accesible y amigable, en el cual el examinado participaba activamente,aún cuando en la fase de recuerdo libre y recuerdo indexado uno, se manifestaban
ansiosos, esta condición disminuía notablemente a través del transcurso de la ejecuciónde la prueba, dado que se sentían reforzados a través de percibir el progreso en su
rendimiento. Sin embargo, aquellas personas en las que no se incrementaba sudesempeño, se pudo advertir cierta inquietud a propósito de la evidencia de su déficit
mnésico, elicitando con ello, preguntas sobre los posibles métodos para recobrar la función
mnémica.
Se observó, además, que el test de procedimiento libre e indexado de Grober y Buscke,efectivamente pesquisaba las alteraciones mnémicas presentes en diferentes sujetos,
corroborando con ello, los estudios realizados sobre la utilidad del uso de índicescategoriales, aprendizaje controlado y especificidad de la codificación.
Del mismo modo, fue posible observar que el test de procedimiento libre e indexado deGrober y Buscke discrimina sobre el rendimiento mnémico en el envejecimiento normal en
contraste con en envejecimiento patológico; ya que los adultos mayores con
envejecimiento normal, son capaces de realizar la tarea cuando se les entregan los índicescategoriales. Sin embargo, en los sujetos con envejecimiento patológico, a pesar de laentrega de los índices categoriales, éstos no fueron significativamente útiles.
La prueba permite distinguir entre déficits mnésicos genuinos o aparentes. Los déficitsaparentes, se asocian por lo general a un uso ineficiente de estrategias, tales como el
menoscabo en la atención, capacidad de procesamiento reducida o deterioro de otrosprocesos cognitivos, los cuales pueden limitar el aprendizaje y la memoria. Los déficits
aparentes pueden ser detectados a través de un adecuado control de procesamiento, elcual induce a un procesamiento semántico y coordina la codificación y la recuperación
para el recuerdo efectivo de la señal. Los déficits genuinos, por el contrario, estánpresentes aún cuando se ha llevado a cabo el procesamiento apropiado. En el adulto

mayor, los déficits genuinos se asocian por lo general con la demencia y se deben al
deterioro de procesos específicos. No se deben a deterioros de procesos involucrados enotras actividades cognitivas las cuales son necesarias pero no suficientes para el
aprendizaje, tal como la percepción inicial de los ítems a ser recordados.
Adaptation of a instrument neuropsychology of evaluation formemory in dementia.
The evaluation of the cognitive operation is fundamental not only for the diagnosis of
pathological conditions, but also for the medical and conductual of patients ofmemoryhandling.
The flaw in the different types by heart, is an important deficit in the patients that withdementia.
The objective of this study of adaptation of an instrument of evaluation neuropsychology ofmemory, was to give emphasis to the encodaje operations and recovery of information,
since numerous works showed that the encodaje operations, the recovery conditions andthe interaction affect in a decisive way the mnesic performance.
The size of the sample it was settled down in 20 individuals. The individuals were evaluatedpreviously, discarding to all of those people that presented indexes of possible psychiatric
alterations (Psychosis, DOC, alcoholism, drogadiction, depression), AVE or some dementiaindicator.
The equiparidad of sexes was stayed, with heterogeneous socioeconomic level andminimum schoolary of four years old (necessary period to acquire the functions of read and
write) and a range etary of 50 to 80 years old.
The obtained results indicate that the translation to the spanish and the adaptation starting
from the manual of the test of the of Memory Free Procedure of Grober and Buscke, sharesthe advantages and results obtained by the Belgian investigators.
Although this study was constituted like an exploratory investigation, with a small sample,without statistical significance, it is necessary to highlight that the obtained results are very
similar to those presented by the Belgian test, what it mentions to an appropriatemethodological process.
We have revised the bibliography that has presented experimental evidences that confirmthe general vision that the acting of the memory if it diminishes like a function of the normal
aging, but that it diminishes more in some situations than in others. Indeed, it has beendemonstrated that the immediate memory of a short sequence of articles diminishes very
little with the age, but that the essential losses are in the work memory. In this last case, the
fellow should not only maintain the information in the mind but rather it should alsomanipulate it, to organize it or to combine it with bigger information that he it enters. In the

situations of the long term memory, the grown-ups people show typically decreases of the
yield although here the size of the effect, also, depends on the task or of their conditions.
On the other hand, the most significant deteriorations related with the age are in the tasks
that few guides provide to code the operations and little support in the recovery. However,the deteriorations always diminish and whet presents, in the moment of the acquisition,
instructions (for example generate precise elaborating, or to carry out small motor tasks),and these are combined with support works, for example, a memory with keys orrecognition in the moment of the recovery.
With regard to the application of the test in the different etary groups, it was observed that
this was an accessible and friendly instrument, in which the one examined participatedactively, even when in the phase of free memory and remind index one, they showed
anxious, this condition diminished notably through the course of the execution of the test,
since they were reinforced through perceiving the progress in its yield. However, thosepeople who their acting was not increased, it could notice notice some anxiety because ofrestlessness of the evidence of its mnesic deficit, generating with it, questions the possible
methods to recover the function mnémica.
It was observed, also that the test of free procedure and index of Grober and Buscke,
indeed it inquiry the mnesic alterations present in different fellows, corroborating with it, thestudies carried out about the utility of the use of index category, controlled learning and
specificity of the code.
In the same way, it was possible to observe that the test of free procedure and index ofGrober and Buscke discriminate over on the mnemic yield in the normal in contrast with inpathological aging; since the oldest adults with normal aging, they are able to carry out the
task when they are given the category index. However, in those subject with pathologicalaging, in spite of the delivery of the category index, these were not significantly useful. The
test allows to distinguish among genuine mnesic deficits or apparents. The apparentdeficits are associated in general to an inefficient use of strategies, such as the damage inthe attention, capacity of reduced prosecution or the deteriorate of other cognitivesprocesses, which can limit the learning and the memory. The apparent déficits can be
detected through an appropriate prosecution control, which induces to a semantic
prosecution and it coordinates the code and the recovery for the effective memory of thesign. on The contrary, the genuine déficits, are still present when it has been carried out theappropriate prosecution. In the oldest adult, the genuine deficits are associated in general
with the dementia and they are due to the deterioration of specific processes. They are notdue to deteriorations of processes involved in other cognitive activities which are necessary
but not enough for the learning, just as the initial perception of the articles to beremembered.
