EPIDEMIOLOGÍA
Corresponden a la segunda causa de infecciones nosocomiales.
Usualmente causa por Staphylococcus coagulasa negativo.
La mayoría de los casos se da en paciente con hemodiálisis y en catéteres de corto plazo
En UCI aproximadamente 80.000 infecciones al año por catéteres.

ETIOPATOGENIA
Microorganismo: vía que alcanzan para llegar hasta el catéter.
Migran desde la piel hasta el sitio extraluminal. Alcanzan el sitio endoluminal 10 – 20%.

La vía que utilizan los microorganismos para alcanzar la
superficie del catéter varía en función del tiempo de permanencia
del mismo
Los catéteres de duración corta (<8 días) se colonizan
por microorganismos de la piel en un 70-90% de los
casos
Los microorganismos migran desde la piel hasta alcanzar la
superficie IV del catéter a través de la fibrina extraluminal
La vía endoluminal, está involucrada en el 10-

cultivo cuantitativo o semicuantitativo.
FlebitisEritema, calor y dolor en el trayecto de una vena
recientemente cateterizada.
Sitio de Salida:
Microbiológico
Exudado que se produce en el lugar de salida con
hallazgo de microorganismo sin o con presentar
bacteremia
Sitio de Salida: Clínico
Edema, rubor, calor y dolor < 2 cm del sitio de salida del
catéter, con exudado purulento, con SIRS, acompañado o
no de Bacteremia.
Sitio de Salida:
Infección del Túnel
Edema, rubor, calor y dolor > 2 cm del sitio del túnel, por
el tracto subcutáneo, acompañado o no de Bacteremia.

INFECCIÓN DEFINICIÓN
Bacteremia: Relacionado a
la infusión
Crecimiento de microorganismos de la infusión y de un cultivo de
sangre periférica sin presencia de otro tipo de infección
Bacteremia:
Asociado al Catéter
Crecimiento > 1 de un microorganismo en un cultivo periférico, en
presencia de sepsis y sin presencia de Bacteremia o fungemia de
origen diferente al catéter.
SIRS que disminuyen 48 horas posteriores al retiro del catéter, con
hemocultivo positivo
Crecimiento de > 15 UFC de cultivo de catéter en una relación 3:1
con respecto al cultivo de sangre periférica.
DIAGNÓSTICO: 2 cultivos positivos:
Cultivo de la punta del catéter + y hemocultivo periférico +
Dos hemocultivos positivos y uno debe ser tomado del catéter y
otro periférico.
Dos hemocultivos positivos que se deben tomar de dos lúmenes
diferentes.

5 – 7 días en casos no complicados.
Retención de catéter hasta 14 días.
Staphylococcus lugdunensis tratar como S. aureus
Staphylococcus aureus
4 a 6 semanas. Si presenta Osteomielitis
llevar a 8 semanas
Si no tiene factores de riesgo y ha mejorado en menos de 72 horas, sin presentar complicaciones, la
terapia puede durar hasta 14 días
Enterococcus faecalis/ Enterococcus faecium
7 a 14 días de tratamiento en paciente
no complicados.

COMPLICACIONES
TROMBOFLEBITIS SUPURATIVA
Cultivo positivos
Visualización de trombo séptico a través de dúplex venoso.
Si es de venas superficiales realizar extracción quirúrgica
Llevar tratamiento mínimo 3 a 4 semanas

ENDOCARDITIS
Realizar ecocardiograma transesofágico a todo paciente que se encuentra con Staphylococcus aureus, infecciones por Candida especies y Staphylococcus lugdunensis.
Realizar control ecocardiográfico de 5 a 7 días.
Retirar en todos los casos el catéter.
Si el paciente no mejora realizar Ecocardiograma.
