From chemistry to aesthetic outcome

The co-founder of botiss biomaterials will share his personal

experience as a scientist that led him to design some of the most

successful biomaterials worldwide. The account of the great

innovations needed to move forward the science of biomaterials

will demonstrate from a unique perspective the relevance,

reliability and efficacy of the botiss collagen and bone products.

Treatment of gingival recessions with

acellular dermal matrices

The treatment of gingival recessions has become an important

therapeutic issue due to the increasing number of cosmetic

requests from patients.

The goal of mucogingival procedures is to obtain complete

root coverage, up to the cementoenamel junction, along with

an increase of soft tissue thickness to maintain the result in the

long-term, reducing the risk of recession recurrence. The use of

a coronally advanced flap in conjunction with a collagen matrix

avoids the need for autograft harvesting, thus reducing patient

morbidity and discomfort. The collagen matrix will support

blood clot stabilization by serving as a scaffold for ingrowing

blood vessels and fibroblasts; within a few months, the matrix

will be completely degraded and the blood clot will be gradually

transformed into new connective tissue, which will be responsible

for the increase in soft tissue thickness. This represents a key

aspect of the long-term success of root coverage. 

The use of a promising porcine acellular dermal matrix for

recession coverage as well as its long-term clinical outcome will

also be described.

Socket preservation:

Matching techniques and technology

Changes in horizontal and vertical dimensions of post extraction

sockets require surgical strategies to counter bone remodeling in

order to facilitate implant positioning at a later stage. During the

presentation an evidence-based decision-making process aimed

at combining the surgical approach with proper regenerative

devices in different clinical conditions will be illustrated.

KLOCKNER® VEGA®: Experiencia y

comportamiento clínico en sus primeros

tres años en el mercado

En mayo de 2015 se presentó el implante KLOCKNER® VEGA® 

como la última incorporación a la familia de implantes

KLOCKNER®  como fruto de su continua actividad de

Investigación + Desarrollo + Mercado. En aquel momento se

resaltaron como virtudes de este producto a) su diseño como

implante de nivel óseo, b) su elaboración en titanio mejorado

de mayor resistencia Optimun® y c) su conexión interna cónica.

El implante VEGA® originalmente incorporaba una superficie

rugosa chorreada. Meses después, KLOCKNER®  incorporaba

al implante VEGA® su nueva superficie rugosa bioactiva Contac-

Ti® que reduce significativamente el periodo de osteointegración.

Con la incorporación del implante Vega, Klockner completaba,

en ese momento, una oferta de tratamiento que incluye implantes

de tipo nivel mucoso (cuello mecanizado) de conexión interna,

implantes de conexión externa, y finalmente, el implante tipo nivel

óseo. En la presente comunicación se hace una reseña de la

aplicación clínica de este implante y de su nueva superficie, con

la presentación de diferentes indicaciones de tratamiento y su

resultado, así como una revisión de sus características.

EL papel de la transición implante-

prótesis en la preservación, creación y

manenimiento del tejido periimplantario

No cabe duda de que en la actualidad, el manejo adecuado

de los tejidos periimplantarios tiene un papel fundamental en

el pronóstico de las restauraciones implantoasistidas. En este

campo, un manejo quirúrgico correcto desde la planificación a

la ejecución presenta un reto en la cirugía plástica mucogingival,

pero no podemos olvidar el papel que cumple la conexión entre el

implante y la prótesis, así como la morfología de la emergencia para

el mantenimiento o recuperación de la morfología periimplantaria.

Es por tanto muy importante discutir el comportamiento de los

tejidos periimplantarios en relación a los aditamentos protéticos

desde la colocación del implante hasta la fase de estabilización a

medio o largo plazo mediante el mantenimiento.

Técnica de reconstrucción alveolar

tridimensional con fines implantológicos

El tratamiento implantológico en localizaciones estéticas tiene

el objetivo de mantener o devolver la estética y función de los

dientes y tejidos periodontales. El manejo de las atrofias óseas y

de tejidos blandos requiere el uso de distintas técnicas de cirugía

plástica periodontal complementarias a la terapia implantológica

en sí, como por ejemplo la regeneración ósea guiada, la

preservación de la cresta ósea y técnicas muco-gingivales con

injertos de tejido blando. El tratamiento de los defectos óseos del

alveolo post-extracción se puede realizar con técnicas clásicas de

regeneración ósea guiada, que prevé el avance de los colgajos,

o con técnicas de preservación ósea, las cuales evitan dicho

avance con un menor invasividad. La reconstrucción alveolar

tridimensional es una técnica propuesta para el tratamiento de

alveolos con amplias dehiscencias que impiden la colocación

del implantes en condiciones adecuadas y que clásicamente

requieren intervenciones de regeneración ósea con amplia

movilización de tejidos y una mayor morbilidad.

The use of a dermal matrix buccally of

immediate implants in the aesthetic zone

The use of soft-tissue grafts buccally of immediate implants leads

to a thicker peri-implant mucosa that is located more coronally

and to a better aesthetic outcome. Subepithelial connective

tissue grafts have been used for many years in plastic periodontal

surgery in the aesthetic zone, but the additional surgical site at the

palate (donor site) leads to a higher post-operative morbidity for

the patient. For this reason, biomaterials such as acellular dermal

matrices have been introduced to replace autogenous soft tissue

grafts. Acellular dermal matrices show good biocompatibility. In

addition, they have shown the potential to thicken oral mucosa

buccally from teeth in an animal study and also at implants in

various clinical studies. The lecture will show results of a clinical

study comparing an acellular dermal matrix versus connective

tissue grafts in conjunction with immediate implants in the

aesthetic zone.
